Closeup of woman's hand over an elderly person's handsThe Memory Clinic is a multi-disciplinary approach to providing early diagnosis and treatment for problems associated with memory loss. The team members work with a physician lead to provide comprehensive care for patients and family members dealing with dementia and other conditions involving memory loss. This information is shared with your primary care physician/nurse practitioner.

Once a referral has been received by your primary care physician/nurse practitioner, an appointment will be made for you to be seen at our Memory Clinic. At this appointment, you and your family members will meet with a team of nurses, social worker, pharmacist and physician for a comprehensive assessment. Each appointment will take approximately 2 hours.  Please bring along a family member or someone who knows you well to your visit to help ensure a thorough assessment.

Prior to your appointment, a package will be sent to you and your family to help you prepare for your visit and provide you with more information about what to expect at your appointment.

Our goal is to help people living with memory difficulties maintain independent living and best quality of life for as long as possible. After your first visit, the Memory Clinic team will continue to work closely with your family primary care physician/nurse practitioner, community agencies, and specialist physicians (if required), with the aim of providing coordinated, ongoing care for you and your family members.
