The Dufferin Area Family Health Team is comprised of a variety of allied health professionals. These include mental health therapists, nurse educators, dietitians, a diabetes educator, a clinical pharmacist, nurse practitioners, a respiratory therapist, a health promoter, and 25 family physicians. Rostered patients will receive the very best care when you need it, as close to home as possible. Vision The Dufferin Area Family Health Team is committed to the betterment of individual and community health through a collaborative, multi-disciplinary primary health care team. Mission Our mission is to improve access to comprehensive multi-disciplinary primary care with an emphasis on collaborative mental health care, focused chronic disease management programs and health promotion programs.
